New Licenses

To apply for a new license, please submit all of the following:
  1. License Application (pdf)
  2. Protocol Summary Form (one for each isotope and procedure) (for research protocols only, animal protocols should be submitted through IACUC) (Webform & pdf) 
  3. Lab diagram indicating areas of use, storage areas, waste storage area, "hot" sink, fume hoods.

License Amendments

To amend an existing license, use the forms listed below:
  1. License Amendment Form (Webform & pdf)
  2. Protocol Summary Form (for research protocols only, animal protocols should be submitted through IACUC) ( Webform & pdf)
  3. Updated lab diagram indicating areas of use, storage areas, waste storage area, "hot" sink, fume hoods.

If the lab is inactivating or leaving the university please refer to, Inactivation Guidelines (pdf).  For amendments involving lab moves or a room removal, please refer to Lab Move Guidelines (pdf). In addition, please refer to Laboratory Close-Out Procedures for instructions regarding other types of hazardous materials.
