Instructions:

This questionnaire will assist lab workers in identifying common safety problems. The questionnaire has eleven sections: Sections I & II cover General Lab Safety concerns and are applicable to all laboratories that handle chemicals; Sections III - XI cover Biosafety Practices.

Review Sections III-XI if you work with any of the following biologically hazardous materials:

human source material (blood, body fluids, tissues);
microorganisms or infectious agents (replication competent or defective);
biological toxins;
recombinant DNA, transgenic animals.
